* What area do you service? We service the entire Fraser Coast and Wide Bay region.
* What services do you provide? We offer Support Coordination & Support workers to assist you with accessing the community. * How will my Support Coordination funding be managed? We will invoice your Plan Manager or you as the participant for the services we provide.
* What payment methods do you accept? We can work with Plan Managed & Self Managed Clients.
* Am I eligible for Support Coordination? You are able to access our services as a Support Coordinator if you have this funding category in your plan. Not everyone receives this funding automatically in their plan.
* What level of Support Coordination will I receive? We can provide both Level 2 & Level 3 (Specialist) Support Coordination - the level you receive will depend on the level of funding provided in your NDIS plan. * What should I do if I receive notice of a Plan Meeting? If you receive a notice of a Plan Meeting you can contact us and we will assist you. You have choice and control over what providers you work with for any service or support you receive. If you aren't happy with our service, please contact us to provide feedback and discuss so we can improve and fix the problem. You can end your Service Agreement Contract with us at any time with 14 days notice.
