Supportive Synergy Services

​We are a local Disability Service offering Support Coordination, Community Access,
In Home Support & Skills & Development Programs to the Fraser Coast Region.

We can work with you to ensure a mix of supports are used to increase your capacity,
assist with utilising your NDIS funding, manage service delivery tasks,
live more independently and be inclusive in your local community.

Our Support Coordinators will work with you to help you find providers,
services and supports that work together with you to
pursue and achieve your NDIS goals.

The services and supports we will link you with will be based on your
personal circumstances, preferences and needs.

If you have Support Coordination or Social & Community Access funding in your
NDIS plan and are Plan Managed or Self Managed we can help you.

Our staff are experienced, professional and caring with years of experience
helping NDIS participants achieve their goals and live independently.
